Job Summary

The Hospitalist Service Clinical Coordinator works with Hospitalist and other providers to facilitate efficient and effective patient care. This position will round with providers and attends huddles as appropriate to identify opportunities for improved patent outcomes, collaborative practice, patient and family satisfaction, and cost effective care. The Hospitalist Service Clinical Coordinator will work collaboratively with the multidisciplinary teams to facilitate desired outcomes, identifies, organizes and analyzes medical data and makes recommendations to achieve desired outcomes and participates in departmental and organizational committees to assist in achieving goals.

Job Description

Essential Functions:

?Participates in strategic planning, research, development and analysis of processes. Contributes to the ongoing development, coordination, and operation of the Hospitalist program. Assists with operational, process improvement and standardization, financial and strategic analysis for the program and presents recommendations. Participates in system wide committees and on projects as required.

?Ensures new ideas are adopted, implemented and participates in communication and educational process to continually support the mission, vision and values of the department, while also working toward accomplishing the goals of the organization. Ensures employees share knowledge, understands decisions and the reasons why decisions are made and implemented.

?Participates and facilitates the integration of policies and practices, to ensure collaboration between the department's advanced practice providers and the organization's nursing, clinical and other medical staff.

?Actively supports, implements, maintains, educates and promotes organizational quality initiatives and goals. Communicates and educates providers and support staff as needed to ensure organizational understanding and compliance with quality standards.

?Serves as the department's point person to obtain necessary information to actively facilitate and coordinate appropriate patient flow through the continuum of care and processes, both intra- and inter-departmentally.

?Receives, documents, and expedites follow up related to patient concerns/complaints in collaboration with other Healthcare members. Performs patient rounds to achieve service recovery and as appropriate, to ensure care is coordinated between all specialist providers.

?Serves as an advocate for the patient and a liaison to patient and the patient's family.

?Possesses in-depth and advanced knowledge of standards of care, practice and Healthcare systems; and uses said knowledge to build strong relationships with medical staff, clinical and non-clinical departments, and all referring providers.

?Uses clinical expertise to interpret data to write comprehensive reports to promote practice standards and improve patient outcomes. Possesses problem solving skills to achieve optimal patient outcomes and identifies opportunities for improvement in care delivered based on clinical outcomes.

?Assists, identifies and participates in orientation of providers to ensure practice standardization, effective throughput, and continuity of care.

?Participates in and collaborates with staff that impact department patient flow, throughput and post discharge follow up.

?All other duties as assigned.

Additional Requirements


Certification - Registered Nurse (RN) - South Dakota Board of Nursing


Experience - 1+ years of Case Management Experience; 1+ years of Discharge Experience; 3+ years of Hospital Experience

Education - Bachelors degree in Nursing

Physical Requirements:

Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
